When combining shear and tension failure mode interaction, do you combine envelope DCR for each, or steel with steel, breakout with breakout, so each mode by itself only?

The interaction equations will be calculated using the governing strength for both tension and shear. For example it may be bond controlling tension and steel controlling shear that would used in the interaction equation. For more information regarding interaction equations, please refer to ACI 318-14 17.6 or ACI 318-19 17.8.
