Spacing of threaded rod installed with epoxy adhesive

I have a (4) bolt cast-in-place anchor bolt pattern where (2) of the bolts are miss-located. The contractor would like to cut off the miss-located bolts and install new threaded rod with epoxy adhesive. What minimum space must be between the unused cast in anchor and the new post installed anchor assuming HIT-HY 200 and threaded rod.
