Rotational Restraint 1 or 2?

When calculating steel failure for standoff anchors with grout, one of the variables is rotational restraint alpha,m = 1 or 2 depending on conditions of the plate to bolt connection. I thought that a base plate would be free to rotate about a anchor if there was no nut on the underside of the plate. What conditions are necessary to determine whether my plate is rotational restrained or not?

Please reference pages 325 and 326 of the PROFIS Engineering Design Guide for further information about rotational restraints in stand-off conditions.
