Post installed or cast in place lifting eye

I am looking at adding lifting eyes to a concrete vault roof beam to help lift valves into pipeline positions. The valves could weigh as much as 12,000 lbs. Are post installed anchors an option? What would be recommended for post installed and cast in place?

Concrete will be typical 4500 psi class A1 concrete.
