On-site Testing of Anchors

I have two different anchorage conditions for a project that I am working on. The first condition is anchorage into existing multi-wythe brick wall. The second condition is anchorage into an existing limestone foundation. I am trying to determine design values for the anchors, and I realize on-site testing may be required for both cases. How have you seen engineers specify the need for on-site testing on the design documents? Do you have sample specification language or general notes language that you can share?
