Our department has been having issues when epoxy/adhesive anchors with CMU. We have been having bond failure in shear. We haven't been able to find anything about this failure mechanism in the technical guide and we were hoping that you could help us understand this failure mechanism and the calculations behind it in the software. I have included an example calculation.

The software is just taking the technical data from HIT-HY 270 here, and applying the appropriate load reduction factors. See Table 3 for shear loads for grout-filled CMU with HIT-HY 270. The software is just labeling it as bond failure, however since the design is in ASD, it could technically be bond or the masonry base material that is failing.
