The firewall at a building separation is (2) parallel rated shaft walls, 2" apart. Where I have a rated door through this firewall, does the 2" gap (annular space btwn walls) at the head & jambs need to be fire stopped?

Yes this would need to be firestopped. We do not have a UL system for this application though, so you would need to submit for an Engineering Judgement.
