Cast in Place Anchor Bolts - Square head vs. hex head

Hi Hilti,


I’m using Profis design software (.pe file attached) and have a question regarding cast in place solutions for hex head vs square head anchor bolts. This may be a stupid question, but I’m having trouble determining why a hex head bolt is failing when a square head bolt checks? The only thing I can think of is that a hex head has less bearing on the baseplate then a square head? Not sure. Hoping for some guidance if possible.


The failure mode is tension in the steel.


Here are the anchor results for square heads (square head stressed at 88%):
