Because of the limitations for post-installed anchors in masonry with respect to head and bed joists, we often provide more holes in the attached steel member than required. Basically two sets of holes, offset in such a way that no matter how the head joints are laid out, one set will meet the spacing requirements. Invariably, we get anchors in all the holes. And we don't get a discrepancy notice from the inspector even though half the anchors are not installed in conformance with the ESR. Is this a problem? I mean besides the failure of the Special Inspector.

Hilti does not recommend installing anchors that do not meet the requirements within the anchor's ESR. Installing anchors that do not meet minimum edge distance or spacing requirements as well as embedment depths, etc. can lead to anchor failure.
