Industry Standard for Concrete Anchors

I've attached pictures of a pool wall aluminum fence that is anchored into a precast concrete wall cap (the cap is attached to the top of a CMU wall with mortar). The cap's dimensions are 24" L x 8.5" W x 4" D. The contractor anchored the fence posts with a 3/8" KWIK Bolt 3 Expansion Anchor with the "B" Length Identifier (2"). During installation, many of the holes were drilled too deep, thus the bolts ended up being driven too deep for a hex nut to be threaded on, as seen in the pictures. The contractor then went back for the "repair" and drilled a 5th hole through the base plate into the concrete wall cap and installed the expansion anchor accordingly, while leaving the existing anchor embedded in the concrete next to it. 1) Is this industry standard, and 2) If not, what should be done in this situation to get the anchor, fence posts, and wall caps to standard.
